Eligible Persons: GCM-Person must have chronic mental health diagnosis and reside in Washington Count. Diversion Program referrals come from DJS only, IHBP all referrals come through DSS. Exclusions to Eligible Persons: N/A. Referral Process: Each program has a specific referral to be completed. Review of all referrals is completed within 24 working hours. If referral appears to meet criteria, contact will be made to the consumer to schedule an Intake assessment within 7 working days. Having completed full assessments and authorization for service received the consumer will be notified of authorization within 24 hours and service begun. Exclusions to Eligible Persons: History of prior psychiatric hospitalizations and/or conditions which result in need for supports beyond the scope of the agency. Accessibility of Service: Handicap accessible for physical, sight, and hearing handicaps. Program offers interpreters for hearing impaired consumers. Referral Process: Call for screening.
