Services: Rental assistance for mental health consumers in independent housing. Specialties: N/A. Eligible Persons: Consumers of mental health services who wish to live independently. Program targets individuals who are homeless. Current criteria also targets those who have been incarcerated for charges less than a felony. Consumers must have a case manager or a counselor from an off-site psychiatric rehabilitation program who provides ongoing, regular contact in the consumer’s home. Exclusions to Eligible Persons: Anyone who is not approved through the application process. Those who have been evicted from an apartment while in Shelter Plus Care or have been terminated from the program due to violations. Accessibility of Service: N/A. Referral Process: Case Manager or Homeless Outreach worker completes the application forms and submits accompanying paperwork to the Washington County Mental Health Authority office. Fees: None, though the tenant is responsible for a portion of rent and utilities based on income. Payment: Tenant pays their share of costs to landlord and utility companies directly. Services: Provides rides to and from mental health activities. Families receive rides to and from hospitals or other placements. Specialties: N/A. Eligible Persons: 1) Washington County Public Mental Health System (PMHS) consumers, 2) Parents or caregivers who accompany PMHS children, 3) Families visiting a hospitalized PMHS relative, if medically recommended. All riders must meet the eligibility criteria and have pre-authorization. Exclusions to Eligible Persons: Anyone who is not approved through the Office of Consumer Advocates, Inc. Referral Process: Mental health program staff, consumer or private practitioners may contact the Office of Consumer Advocates at 301-790-5054.
